I found the missing icon error inside the log:

Ignore the filepath, the function is also checking mod directories.

In this cas i would say, the icons are missing inside the modfolder.

But i added a filecheck to the total mayhem mod and all files are present:

I only see 2 reasons why this happens.
1. The icon directory inside the Total Mayhem root folder was moved inside another folder.
2. FAF gamefiles are changed and the icon support is broken.

are you playing the coop version of the game ?
If yes, can you start a normal FAF game and test the icons in a scirmish play ?

I enabled only two mods. Total Mayhem 1.36 and T3EngStations in the game user/selection + options lobby

you posted the "forever.log" of the FAForever client, but i need the "game.log" from the game itself.

But you say you used the old black ops mod ? The old Black Ops is NOT compatible to the FAForever game patch.
Please search in the Vault for:

BlackOpsFAF-ACUs
BlackOpsFAF-Unleashed
BlackOpsFAF-EXUnits


These versions are made for FAF.

It is interesting, that all unit icons DO show up in the [restrictions] Unit Manager in the lobby.
I am also testing using in solo mode "%programdata%\FAForever\bin\ForgedAlliance.exe"

You are right, FAF has its own Icon support, and if you enable black ops global icon support it will break.

You could do us a favor and post your game.log.

I realize FAF has Black Ops Icon support built in - but I've tried having that mod in the mods folder and NOT having it.
This test appears to have no effect.

I have also tested NOT enabling any mods, and then, one by one, trying to enable each "adds units" mod by itself, and I still see "Place holder"

The actual new units or buildings do get built, and their graphics appear to be fine.

Note: all mods are in the "...Documents\My Games\Gas Powered Games\Supreme Commander Forged Alliance\Mods" folder.

Any suggestions?
