I got the same a few times trying the campaign missions (1 player but through the lobby coop mode)

i had formed a squad with another player.
he clicked attack on planet 96 (we had less than 50% on the planet)
for both of us, the game started, but we ended up in the forged alliance main menu.
after that the planet showed its being defended by someone.
not much to say.
