I see some with a percentage, some without. Are the ones with a ?% an unranked game?

I think "?" means that there is an uneven number of players at that time, so a balance percentage cannot be meaningfully calculated.

To my knowledge, game quality display, and rated or unrated, are entirely independent of each other.

I guess a follow up question - any easy way to know if a game is ranked or not from the pick list? (assuming it is not one of the specialty mods)

not, most of the survival maps are not but there is no fast way to find it out, you shouldnt care so much

While there isn't an easy way to see from the lobby, the following make a game unranked: any non-ui mods enabled, any unit restrictions, cheats on, any victory condition other than annihilation, custom games under 120s, or fighting against the AI (I assume, as I've never wasted my time doing it).
