Assuming you are using onboard sound:
1.Restart PC.
2. Go into bios. Disable onboard sound. Save and boot
3. Start FA (run the game, not just the lobby)
4. Close game and reboot.
5. Go back into bios and enable sound.
6. Reboot and run FA. Your sound ought to work properly now.
It appears that Directsound saves some config info related to FA in the registry (I haven't been able to figure out what it does precisely). It gets confused and buggers up sound prioritizing or somesuch. Rebooting doesn't always help because the settings persist in the registry. For the same reason even reinstalling the game doesn't help (unless maybe you choose a different install path, but that's speculation).
Point being, it's seems to be a DirectX issue and not necessarily a supcom issue, but there is some interaction going on behind the scenes that effs it up.
Disabling sound hardware makes Directsound reset its app settings. A sound driver reinstallation has the same effect, but that's a last resort.
I can replicate this behaviour time and again by alt-tabbing out of FA and running WMP to play my own music and then alt-tabbing back into FA. The solution I outlined always fix it for me, but I've gotten into the careful habit of starting WMP and playing my zax before starting FA.
