Hi,

Friend and I are trying to play a game with teams, but when we start the game, the teams don't always match up with the settings in the lobby (sometimes they do?)

Are we doing something dumb? (attached test lobby and settings with AI, as it does the same thing). It's been doing this a long time, but most of the time we were able to get a game working, but it's basically become the reason we stopped playing FAF. Originally we thought it was one of the specific mods, but we have turned off all the mods and only using Vanilla FAF and it still displays the issue.

Have tried completely reinstalling FAF and the game files as well.

There is a setting called Spawn. It's the first visible setting in your "Settings" screenshot. You have it on "Random" which will make the spawn random. If you change that option to Fixed you can get fixed spots.

The random spawn is fine, I don't care of the locations where we spawn, other than the person I pick to be on my team should be on my team. Do the spawn locations now also dictate who your allies are?

Note in my lobby screenshot, I have the UEF AI set as my ally, but when the game starts, my ally ends up being the Cybran AI?

Non-fix spawn will mix up the players. The teams are decided by the slots.
In your example you have slot 1+2 vs slot 5+6. The random mixes up the players but the slot 1+2 will still be team 1 and slot 5+6 will still be team 2.

This option is intended to have set teams in terms of e.g. top vs bottom with random team members. And as far as i know this has always worked that way.
